Job Description:
Overview Full-time, Monday - Friday, no weekends or eves. Manage
chronic conditions of high-risk geriatric patients in their homes
across North Houston with a supportive team (MA, RN Care Coord,
Social Workers, Schedulers) and a company car. HarmonyCares is a

in select markets Responsibilities The Advance Practice Provider
(APP) we seek are those who have directly related experience in
home health, geriatrics, hospice and primary care. The APP who
wants to practice medicine with a focus on patient care, not
volume. We want our clinicians to take the time needed to truly
address the patient’s needs. Qualifications Licensed as a NP or PA
in the State where HarmonyCares Medical Group office located
Federal DEA License/eligibility Geriatric training/experience
preferred Active CPR/BLS Certification Board Certification Pay
